Adelfi Medical Billing Solutions is a growing medical billing and revenue-cycle management company serving 65 healthcare clients across the country. Our team of over 50 U.S. employees keeps our clients' practices running by handling billing, collections, credentialing, and the operations behind them - and we're still growing. As we scale, we're building out our leadership bench, and this is our first dedicated Human Resources Manager role: an opportunity to own and shape the people function from the ground up.
The opportunity
We're looking for a hands-on, detail-driven HR Manager to own payroll and the full employee lifecycle for a distributed, multi-state workforce. You'll be the go-to person for everything people-related: running an accurate biweekly payroll, recruiting and onboarding new team members, administering benefits, keeping us compliant across multiple states, and helping build the kind of workplace people want to stay at. This is a high-trust, high-autonomy role reporting to the Director of Business Operations and working closely with the owners.
What you'll do
Payroll (in-house): Run accurate biweekly payroll for full-time employees and 1099 contractors; manage multi-state payroll tax registrations and filings (we currently operate in 11 states plus local jurisdictions); ensure timeliness, accuracy, and compliance.
Recruiting & hiring: Own full-cycle recruiting - sourcing, screening, interview coordination, reference checks, and offer letters - partnering with hiring managers to bring in strong talent.
Onboarding: Deliver a smooth onboarding experience: new-hire paperwork, I-9s, employee files, and coordination of equipment and system access with our operations team.
Benefits & leave: Administer employee benefits and lead annual open enrollment with our broker/carriers; manage PTO, bereavement, and other leave requests.
Compliance & policy: Maintain the employee handbook and HR policies; keep the company compliant with federal and multi-state employment law, ACA, and record-keeping requirements.
Compensation: Research and recommend annual cost-of-living adjustments and respond to salary-review requests with market data, in partnership with leadership.
HRIS & records: Own and maintain the HRIS/payroll platform (Paychex) as the single source of truth for employee data, time off, and pay.

What you bring
5+ years of progressive HR experience, including hands-on payroll processing.
Working knowledge of multi-state payroll and employment compliance.
Experience with an HRIS/payroll platform (Paychex strongly preferred).
Strong organization and discretion - you handle confidential information with care.
Clear, warm communicator who can support employees and advise leadership alike.
SHRM-CP/SHRM-SCP or PHR/SPHR certification preferred.
Experience in healthcare, medical billing, or RCM - and with distributed teams - is a plus.
